Monthly Fees
Lupine Family Medicine is a direct care clinic. This means that we do not bill health insurance companies for all that we do for our patients.
Instead, we are directly paid/hired by our patients with a monthly membership fee which effectively bypasses the hassles that occur with having to deal with third party payers. This allows your physician to attend to you fully and promptly with honest, transparent pricing.

$50 first child
$25 for each additional child
Your monthly membership includes:
30-60 minute consultations
in-person or virtual visits
email/call your physician directly
no copays, enrollment fees or advance payments required
no surprise/hidden fees
in house labs and tests are included in the monthly fee
heavily discounted send-out labs
heavily discounted common medications directly dispensed from your physician
SPECIALIZED SERVICES $125/month, includes primary care
(once stabilized, typically 12 months - return to usual monthly membership)
■ Suboxone induction/treatment
■ Controlled medication management
■ Alcohol use disorder - Naltrexone, Acamprosate
■ Management of polypharmacy
ONE TIME VISITS without a monthly membership
■ Sports physicals $50
■ Pre-employment physicals $150
■ CDL/DOT exams $150
■ Medical cannabis authorizations $200
■ Veterans disability examinations - contact the VA to get these arranged at my office
■ Immigration physicals $500 includes lab tests, other tests and vaccines not included

Patients may cancel their membership anytime. If they decide to return to the practice, they will need to pay a re-joining fee of $350.
